Hospice Care

Skilled nursing care allows for medical services to be provided where it’s most comfortable – at home. Covered by Medicare, MediCal, CenCal, and most private insurance, our registered nurses offer a full range of home health services.

Physical Therapy

Physical therapy services are crucial to help individuals regain or strengthen functional abilities. Our physical therapist works closely with an individual’s doctor to develop a plan, set goals, and work toward achieving them. The therapist can also provide training and education on using adaptive equipment.

Occupational Therapy

Our occupational therapists develop personalized programs to help individuals live the most independent lifestyle possible. This includes incorporating any adaptive equipment that will make daily activities easier to accomplish. Through occupational therapy, functionality can be improved in a number of key areas, such as eating, dressing, personal hygiene, exercising, and more.

Speech Therapy

After a traumatic brain injury, stroke, or other health condition that impacts speech and/or swallowing, in-home speech therapy can help individuals regain functionality to the fullest extent possible. One of our speech therapists will create a personalized care plan to focus on improving speech, swallowing, and more.

Post-Hospital Care

Post-hospital care is crucial after a surgical procedure or hospitalization to ensure faster healing and recovery. Our skilled home health care team can provide a wide range of services to ease the transfer home from the hospital, monitor for any changes in condition, and prevent a re-hospitalization.

Wound Care

Our skilled home health team can help promote wound healing and reduce the risk of infections or other complications in the comfort of home. Some of the types of wounds we can care for include ulcers, pressure sores, neuropathy sores, burns, surgical wounds, and more.

Non-Medical Home Care

We are pleased to help those in need of durable medical equipment (DME), through the kind donations of our community neighbors and partners. Our DME loan closet is open and available to you with items such as:

  • Toilet seat risers and stabilizers
  • Shower chairs
  • Knee cruisers

Getting Started With Home Health or Hospice

If a physician has ordered home health or hospice services for a loved one, or if you believe a loved one could benefit from our services, we’re here to help! Referrals to home health and hospice typically are made by a patient’s physician and can also occur after hospitalization or a skilled nursing stay. It’s important to know that your loved one has the right to request a referral to the home health or hospice provider of their choice.

If you have any questions or need any assistance with the referral process, please reach out to us at 805-742-4514 and we can help you initiate services. Once the referral is received, we will call you to schedule an initial assessment with one our nurses or physical therapists.
