Home Health Services

Home health care allows for necessary medical services to be provided where it’s most comfortable – at home. Covered by Medicare, MediCal, CenCal, and most private insurance, our professional home health care team is comprised of nurses, physical, occupational, and speech therapists, a lymphedema specialist, and home health aides. We offer a full range of medical and social services, such as:

Hospice & Palliative Care

One of the most challenging and emotional journeys we face is when someone we love has a life-limiting diagnosis. It’s a time when having a strong support system is crucial. It is our great honor to walk alongside your family during this time, providing comfort and assistance every step of the way.

Our hospice services are covered 100% by Medicare and most insurance and there will never be any out-of-pocket costs regardless of insurance plan. Our dedicated hospice and palliative care staff provide holistic care for a range of physical, emotional, and spiritual needs. We are proud to offer a patient- and family-centered approach to end-of-life care.

With a team of highly qualified clinicians and staff we develop a customized plan that focuses on caring for the whole person, assisting with:

  • Therapeutic music
  • Medication and symptom management
  • Personal care and comfort
  • Respite care for family caregivers
  • Nutrition guidance and assistance
  • Coordination of hospice-approved medications, medical supplies, and equipment

Led by our hospice medical director, each person’s care team includes a registered nurse, hospice aide, medical social worker, spiritual counselor, bereavement counselor, and hospice volunteers. We’re here for you around the clock to ensure your needs are fully met.

Non-Medical Home Care

Whether the need is for just a little help around the house or a higher level of support to ensure safety and wellbeing, our customized non-medical home care services, offered by our sister agency, Superior Senior Home Care, are the ideal solution to help older adults maintain independence.

We begin by meeting with you in the comfort of your home to develop a care plan to address any challenges being faced. We than carefully select just the right caregiver to assist with services such as:

  • Companionship to ensure that older adults stay socially active, leading to better health and wellbeing
  • Personal care for safe showers or baths, dressing, and more
  • Homemaker services for a clean, orderly, and safe home environment

Medical Loan Closet

We are pleased to help those in need of durable medical equipment (DME), through the kind donations of our community neighbors and partners. Our DME loan closet is open and available to you with items such as:

  • Wheelchairs

  • Crutches
  • Toilet seat risers and stabilizers
  • Shower chairs
  • Knee cruisers
  • And more

Getting Started

If a physician has ordered home health or hospice services for a loved one, or if you believe a loved one could benefit from our services, we’re here to help! Referrals to home health and hospice typically are made by a patient’s physician and can also occur after hospitalization or a skilled nursing stay. It’s important to know that your loved one has the right to request a referral to the home health or hospice provider of their choice.

If you have any questions or need any assistance with the referral process, please reach out to us at 805-742-4514 and we can help you initiate services. Once the referral is received, we will call you to schedule an initial assessment with one our nurses or physical therapists.
